The van will run from 1 to 6:30 p.m. from the South Santa Rosa Service Center, 5841 Gulf Breeze Parkway in Gulf Breeze, to the health department’s Milton location at 5527 Stewart Street. Space is limited and reservations are required.  Parents may reserve seats by calling Tom Turman of Ferris Hill Baptist Church at 850-261-7470.

Children must be accompanied by a parent or legal guardian (documentation of guardianship is required) and parents are asked to bring a copy of the child’s immunization record.  Immunizations are provided by the health department free of charge to children through the age of 18 years and children must be up to date on all required vaccinations in order to attend public school in the State of Florida.
